Kathleen Pendleton, MS, MJ, CPHRM, CPPS, CPHQ, is currently the Director of Just Culture and Patient Safety Projects at Wellstar Health System in Marietta, Georgia. She is responsible for the development, deployment, and sustainment of Just Culture across the Wellstar enterprise. Kathleen has extensive knowledge and experience as a healthcare leader in Clinical Risk Management and Patient Safety. She has a master’s degree in healthcare risk management from Rosalind Franklin University and a master’s degree in healthcare law from Loyola University Chicago School of Law. She is certified in healthcare risk management, patient safety, and healthcare quality.

Steve Mrozowski, MHA, CPPS, FACHE, serves as a Partner in Clinical Transformation and leads External Peer Review, Patient Safety, and high reliability offerings. He brings more than 25 years of healthcare experience in prehospital services, quality, safety, and high reliability organizing and education. In his role, Steve is focused on assisting organizations in optimizing their capabilities to deliver harm-free, patient-centered, and high-quality care. His expertise and passion are most prominent in the principles of patient safety structures, Just Culture, root- and apparent-cause analysis, lean/six-sigma, and high reliability. Prior to joining Chartis, Steve served as Senior Director of Safety & High Reliability at Cleveland Clinic, where he led the enterprise-wide patient safety program, high reliability organizing activities, environmental health and safety, and Cleveland Clinic’s component Patient Safety Organization. Previously, he was the inaugural Director of Patient Safety at the Penn State Health Milton S. Hershey Medical Center and led the organization to a reduction in serious safety events by more than 75% during his tenure. Steve’s clinical experience in emergency medical services spans more than 25 years. In addition, Steve speaks nationally on topics of patient safety, high reliability, and harm reduction. He actively participates nationally with the American College of Healthcare Executives (ACHE), where he is a Dolan Scholar and has served local chapters as the Diversity, Equity, and Inclusion chairperson. He was recently honored with an ACHE Regent’s Award for Leadership. Steve also serves as core Patient Safety Faculty for the American Society for Healthcare Risk Managers (ASHRM). Steve earned his undergraduate degree in emergency medicine and completed Paramedic training at the University of Pittsburgh and his Master of Health Administration from Penn State University. He also completed graduate education in Organizational Development from Penn State. Steve is a board-certified Fellow of the American College of Healthcare Executives, a Certified Professional in Patient Safety, and a Certified Professional in Human Factors in Healthcare.

Tatum O’Sullivan, RN, MHSA, CPHRM, DFASHRM, CPPS, BSN, is the Healthcare Loss Control Expert for Munich Re Specialty – North America. She was recently the Director of Safety, Risk, and Patient Family Relations at Mass General Brigham Medical Group in Massachusetts. She is a Certified Professional in Health Care Risk Management (CPHRM) and Distinguished Fellow of the American Society for Health Care Risk Management (DFASHRM). Tatum is a dedicated member of the American Society for Health Care Risk Management (ASHRM), where she has served and chaired committees. She was previously elected to the advisory board and nominating committee and was the 2024 President of ASHRM. She has been on the Board of the Northern New England and Massachusetts chapters affiliated with ASHRM serving as Secretary and President for both. Lastly, she is a Certified Professional in Patient Safety (CPPS) and a faculty member for the ASHRM patient safety certificate program. She has presented on risk management and patient safety strategies locally, nationally, and internationally. She was a contributor to several ASHRM workbooks and playbooks. Tatum was a co-author to the 2025 ASHRM whitepaper, Strategies for Promoting Risk Professional Resilience.
